We are seeking a skilled and compassionate Nurse Clinician to join our team at Vivo HealthStaff. As a key member of our interdisciplinary team, you will be responsible for providing high-quality nursing care to our participants at the Day Health Center.
Key Responsibilities:
• Assess participants' needs and develop individualized care plans
• Implement, direct, and evaluate nursing care for participants
• Teach, supervise, and counsel participants, geriatric aides, and family members on nursing care
• Collaborate with the interdisciplinary team to provide comprehensive care
• Participate in team planning, problem-solving, and process improvement
Requirements:
• Graduate of an accredited college or university with an associate or baccalaureate degree in nursing
• Current California Registered Nurse license
• California Public Health Nurse certificate desired
• Experience in public health nursing desired
• One-year experience working with the elderly or frail population required
• Ability to work as part of a multidisciplinary team
• CPR certification
