Good Shepherd Hospice is in search of a dedicated Licensed Practical Nurse (LPN) to provide exceptional care to our patients and their families within the Wichita area and its surroundings.

The Hospice LPN will conduct visits to enhance the nursing care provided to our patients and their families.

  • PRN position available
  • Previous hospice experience is advantageous
Key Responsibilities

  • Receives patient assignments from the Patient Care Supervisor.
  • Initiates communication with the RN Case Manager, hospice team members, and other agencies as necessary to ensure optimal care and resource utilization for the patient and family.
  • Develops communication and bereavement skills.
  • Maintains ongoing communication with the attending physician regarding patient and family care.
  • Contacts families of newly assigned patients and maintains regular communication as requested or desired by the family.
  • Gathers information on physical, psychological, social, and spiritual factors that may affect the health status of the patient and family, participating in the Plan of Care.
  • Keeps accurate and current patient records, ensuring that problems, plans, actions, and goals are clearly documented and updated as necessary. Submits paperwork as required by Medicare and Good Shepherd Hospice policies.
  • Takes responsibility for coordinating the physical care of the patient by educating primary caregivers, volunteers, employed caregivers, or by providing direct care as appropriate.
  • Instructs the patient and caregiver on:
a. Medication administration, including recognizing side effects and adverse reactions. b. Personal care and oral hygiene practices.

c. Assistance with lifting, moving, ambulation, and exercise.

d. Disaster and Safety Management protocols.

e. Preparations for the events preceding death.

  • Informs the RN Case Manager of any unusual or potentially concerning patient or family issues.
  • Participates in Hospice team meetings and other patient conferences, including care plan meetings, as deemed necessary by the Patient Care Supervisor.
  • Provides appropriate support during the time of death.
  • Shares in delivering 24-hour, seven days a week coverage to patients and families.
  • Engages in individualized studies to enhance understanding of hospice philosophy.

  • Develops proficiency in pain management through individualized reading, in-services, or seminars.
Skills Knowledge and Expertise

  • A minimum of one year of experience in hospice, home health, or geriatric care is preferred (but not mandatory for the right candidate).
  • Must exhibit high integrity, including the maintenance of confidentiality regarding all patient records.
  • Works collaboratively as a team player, following directions and adhering to established care plans.
  • Demonstrates a commitment to the hospice philosophy of care.
  • Must successfully pass background checks.
  • LPN-graduate of an accredited LPN program.
  • Current state license as a Licensed Practical Nurse.
  • Valid driver's license.
  • Valid Nursing License.
  • Strong organizational and self-management skills.
  • Must successfully pass background checks.
