Key Responsibilities- Develop and implement comprehensive care plans in collaboration with patients, families, and healthcare providers.
- Provide skilled nursing care, including wound care, infusion therapy, and catheter management, as needed.
- Educate patients and their families on disease management, medication adherence, and self-care techniques.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans.
- Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
- Active RN license in the state of application.
- Valid CPR certification.
- 1 year of experience in a healthcare setting, preferably in home health or a related field.
