Nurse Practitioner

The Nurse Practitioner is a highly skilled and patient advocate in a particular specialty or subspecialty of nursing practice. This position manages a group of clinically complex or resource intensive patients, providing expert care and support throughout their treatment journey.
Responsibilities and Duties- Perform and document comprehensive medical and psychosocial health histories and examinations.
- Gather data, conduct initial history and physical examination, and formulate and implement treatment plans in collaboration with the physician.
- Order and interpret diagnostic studies related to the patient's plan of care and response to treatment.
- Provide follow-up care on an inpatient and outpatient basis.
- Initiate consultation with specialists as indicated.
- Recognize oncology-related problems and initiate interventions.
- Identify treatment-related toxicities and initiate interventions.
- Demonstrate expertise in symptom management.
- Assess ill patients and develop a plan of care in collaboration with MD.
- Monitor and adjust patient medications and treatments according to protocols.
- Diagnose and treat common oncology-associated illnesses.
- Manage telephone calls related to emergency situations, symptom management, and psychosocial and family concerns.
- Provide continuity of care across settings (hospital, home, home health, skilled nursing, hospice, and rehabilitation).
- Supervise chemotherapy administration in the office setting.
- Collaborate with physician and chemotherapy infusion nurse to schedule and administer chemotherapy.
- Write chemotherapy orders and evaluate protocols in collaboration with physicians.
- Provide psychosocial counseling and support to patients and families and make referrals to social work or community agencies as necessary.
- Evaluate the effectiveness of integrative therapies and their use in complementing medical treatment.
- Demonstrate knowledge of cancer risk factors, diagnoses, disease processes, and treatment options.
- Evaluate patient intervention outcomes.
- Coordinate discharge plans, follow-up, home health, and outpatient prescriptions.
- Participate in daily huddles/conferences with the team to facilitate communication and coordination of care.
- Provide survivorship care to patients who have completed chemotherapy.
- Masters of Science in Nursing (MSN) from an accredited nursing school and successful completion of a Nurse Practitioner program.
- 3-5 years of professional nursing experience in a hospital or clinical setting with 1-2 years in a nurse practitioner role or 1-2 years in an Oncology Nurse Practitioner Role.
- RN state licensure and current registration with the State Board of Nursing in practicing state.
- Current state licensure as a Nurse Practitioner.
- AOCNP preferred.
