Intake Coordinator

We are seeking a highly skilled Intake Coordinator to join our team at Freedom Home Health and Hospice Care Services Inc. The successful candidate will be responsible for managing all aspects of the patient intake process, including establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and ensuring seamless transition of patients to home care.
Key Responsibilities- Perform daily patient referral and intake operations, ensuring compliance with state, federal, and CHAP referral/intake regulatory requirements.
- Implement improved work methods and procedures to ensure patients are admitted in accordance with policy.
- Establish and maintain positive working relationships with current and potential referral sources.
- Coordinate patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers.
- Ensure maximum third-party reimbursement through direct oversight of insurance verification and authorization processes.
- Build and monitor community and customer perceptions of Freedom Home Health and Hospice Care Services Inc. as a high-quality provider of services.
- Maintain comprehensive working knowledge of Freedom Home Health and Hospice Care Services Inc. contractual relationships and ensure that patients are admitted according to contract provisions.
- High school graduate or equivalent.
- Two (2) years data entry experience, preferably in home health or similar operation.
- Previous healthcare-related experience.
- Ability to type 50 words per minute.
- Word processing skills.
- Personal computer skills.
- Demonstrates good communication, negotiation, and public relations skills.
