Job Summary

We are seeking a skilled and caring Home Health Care Professional to join our team. As a Home Health Care Professional, you will be responsible for providing personalized care to our clients in the comfort of their own homes. Key Responsibilities
  • Provide high-quality care to clients in their homes, including assistance with daily living activities, medication management, and mobility support.
  • Develop and implement personalized care plans that meet the unique needs and goals of each client.
  • Collaborate with healthcare professionals and family members to ensure seamless care coordination.
  • Maintain accurate and detailed records of client care, including progress notes and medication administration.
  • Participate in ongoing training and education to stay up-to-date on best practices and industry developments.
Requirements
  • 1,000 hours of long-term care experience with geriatric or chronically ill persons (exceptions may be made for recently licensed CNAs or PCAs).
  • Must be physically able to perform the duties of this position.
  • Must have available reliable transportation and a valid driver's license.
  • Pass a national and state-level background check and a drug screen.
  • This role is classified as a safety-sensitive position and subject to applicable state and federal regulations.
