Licensed Practical Nurse

The Licensed Practical Nurse (LPN) plays a vital role in ensuring the highest level of physical, mental, and psychosocial well-being for our residents. As a key member of our care team, you will provide direct care, administer medications, and promote a safe and supportive environment.
Key Responsibilities- Provide individualized care to residents, administering medications and treatments as prescribed by physicians.
- Act as an advocate for residents, maintaining their dignity and confidentiality.
- Monitor resident vitals, completing assessments as assigned, and communicating changes to the care team.
- Collaborate with the interdisciplinary team to ensure seamless care delivery.
- Assist with dining room activities, supporting residents with eating and hydration needs.
- Participate in the admission process, ensuring a smooth transition for new residents.
- Perform focused assessments of residents with acute changes in condition, reporting findings to the medical team.
- Attend resident care conferences as needed, contributing to the development of individualized care plans.
- Provide guidance and support to nursing assistants and other team members.
- Current licensure as a Licensed Practical Nurse in the State of Minnesota.
- One or more years of experience in long-term care, preferably in a skilled care setting.
- Current certification in CPR.
- Strong communication and problem-solving skills.
- Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
- Proficiency in Microsoft Office and other relevant software applications.
The work environment is indoors, with regular exposure to residents, family members, and team members. You will be required to work a flexible schedule, including overtime, nights, and weekends as needed.
