About the Role

This is an ongoing opportunity for a Physical Therapist to provide home-based care services to patients. As a member of the LHC Group team, you will have the autonomy to work independently while being supported by a collaborative team.

Responsibilities
  • Evaluate patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions.
  • Develop and implement treatment plans to improve patient well-being.
  • Collaborate with healthcare professionals to ensure comprehensive care.
  • Provide education and support to patients and their families.
Requirements
  • A current Physical Therapy licensure in the state of practice.
  • Current CPR certification required.
  • A reliable vehicle and valid driver's license.
