Job Summary:
We're seeking a skilled Primary Care Nurse Practitioner or Physician Assistant to join our team as a Senior Health clinician. This per diem, hourly role involves working with Senior Health patients (ages 65+) who require a broad range of medical services, including acute, chronic, and well visits. You'll work closely with in-office teammates and virtual clinical teams to deliver high-quality patient care, while utilizing your specific clinical training and expertise to perform in-office procedures.
Key Responsibilities:
- Conducting in-office and telehealth visits for Senior Health patients
- Collaborating with colleagues via daily huddles and virtual clinical teams
- Utilizing evidence-based clinical research to inform patient care decisions
- Performing in-office procedures and participating in ongoing education and training opportunities
Requirements:
- At least 2 years of primary care experience required
- Current licensure in the state of Colorado or ability to obtain licensure
- Excellent communication and interpersonal skills, with a passion for human-centered primary care
- Ability to thrive in a fast-paced, mission-driven organization that utilizes data to improve patient outcomes
- Board Certification

The estimated annual salary for this role is $75,000 - $100,000 based on factors such as practice experience and patient care schedule.
