Job Summary

Acentra Health is seeking a skilled Clinical Evaluator to join our team. As a Clinical Evaluator, you will conduct face-to-face assessments to facilitate appropriate placement and next level of care in a variety of settings.

Key Responsibilities
  • Conduct thorough screenings, evaluations, and determinations to ensure accurate assessments.
  • Assess and analyze psychological, emotional, behavioral, and physical functioning to develop informed level of care recommendations.
  • Develop and implement effective care plans to improve patient outcomes.
  • Collaborate with healthcare professionals, patients, and families to ensure seamless care transitions.
  • Stay up-to-date on current regulations and research related to long-term care, mental illness, medical conditions, and geriatrics.
  • Utilize technology to efficiently manage and track assessment activities.
Requirements
  • Minimum 3 years of experience assessing individuals with mental or psychiatric disorders.
  • Active unrestricted licensure in Washington in a healthcare or related field.
  • Graduation from an accredited college with a relevant degree program.
  • Expertise in assessing and analyzing psychological, emotional, behavioral, and physical functioning.
  • Working knowledge of Preadmission Screening and Resident Review (PASRR) requirements.
  • Knowledge of psychiatric disorders and standards of practice related to mental health, geriatrics, and other disorders.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
