Job Title: Registered Dietitian

We are seeking a skilled Registered Dietitian to join our team at CommuniCare Health Services. As a Registered Dietitian, you will play a vital role in assessing the nutritional needs of our residents and maintaining accurate records and Plans of Care.

Job Summary:

The successful candidate will be responsible for:

  • Completing quarterly and annual nutrition assessments on all residents, following the Nutrition Care Process.
  • Developing and implementing nutrition interventions, documenting medical and nutrition-related data, providing individualized education, and monitoring daily intakes.
  • Actively participating in quality assurance and performance improvement programs, care plan and nutrition at risk meetings, and communicating with other interdisciplinary team members.
  • Monitoring quality and accuracy of food served and acceptance of food and beverage by residents during meal rounds.
Requirements:

To be considered for this position, candidates must be licensed and registered as a Dietitian in the State of employment. Long Term Care/Geriatric nutrition experience is preferred.
