Job Summary:
The RN Case Manager at Temple University Hospital plays a vital role in facilitating the patient's progress through the acute episode of care in an efficient and cost-effective manner. This position requires strong leadership capabilities, knowledge of theories, principles, and concepts related to nursing practice, and experience in acute care case management or utilization management.
Key Responsibilities:
- Collaborate with patients, families, physicians, and the interdisciplinary team to develop and implement individualized care plans.
- Monitor the level of care and initiate evidence-based protocols and order sets according to the patient's admission diagnosis.
- Facilitate the patient's navigation through the acute care episode and propose an appropriate transition plan within 48 hours of admission.
- Serve as an advocate for the patient and family throughout the entire acute episode of care.
Requirements:
- Bachelor's Degree in Nursing (BSN) or a combination of education and experience in lieu of a BSN with a commitment to obtain a BSN within 2 years from date of hire.
- 1 year of experience in acute care case management or utilization management at an acute care facility and/or insurance company in either utilization review (UR) or case management (CM).
- General experience with knowledge of theories, principles, and concepts related to nursing practice and strong leadership capabilities.
- General experience and knowledge of InterQual criteria (preferred).
- RN-LIC - PA Registered Nurse License (required).
- CCM - Certified Case Manager (preferred).
