Key Responsibilities:- Conduct initial assessments and develop comprehensive transition plans for patients
- Participate in multidisciplinary rounds to ensure coordinated care
- Perform utilization reviews to optimize resource allocation
- Advocate for patients and their families, addressing their unique needs and concerns
- Delegate tasks to Case Management assistants as needed
- High degree of skill in written and oral communication, with the ability to interact effectively with patients, families, and healthcare teams
- Agility in a fast-paced environment, with the ability to adapt to changing circumstances
- Computer competency, with experience in electronic health records and other relevant software
- Ability to complete daily job responsibilities within normal hours, with flexibility to work varied shifts as needed
- Culturally competent, with a commitment to diversity, equity, and inclusion
- Three or more years of clinical experience in a healthcare delivery system, with a focus on case management or a related field
- Completion of an accredited Nursing program, with a current Nursing License in the State of MA
