Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team. The successful candidate will provide exceptional care to clients served by the Department of Behavioral Health and Developmental Disabilities (DBHDD). The LPN will work collaboratively with a multidisciplinary team to deliver personalized and client-centered healthcare.

Key Responsibilities
  • Conduct thorough assessments of clients' physical and mental health
  • Collaborate with healthcare professionals to develop individualized care plans
  • Monitor and update care plans as needed to address evolving client needs
  • Administer medications as prescribed by healthcare providers
  • Monitor and document medication effects, making adjustments as necessary
  • Maintain accurate and up-to-date client records, ensuring compliance with regulatory standards
  • Document all interactions, observations, and interventions in a timely manner
  • Work closely with physicians, psychiatrists, social workers, and other healthcare professionals to coordinate comprehensive care
  • Participate in team meetings to discuss client progress, challenges, and adjustments to care plans
  • Provide education to clients and their families on health conditions, treatment plans, and self-care strategies
  • Foster a supportive and informative environment to empower clients in managing their health
Requirements
  • Experience in nursing, preferably in behavioral health or developmental disabilities settings
  • Compassionate and patient-focused approach to care
