Nurse Practitioner

We are seeking a skilled Nurse Practitioner to join our team on a PRN/Part Time basis. As a Nurse Practitioner with Matrix Medical Network, you will conduct home risk assessments, medical history reviews, and provide health education to patients in their homes. • Conduct home risk assessments and medical history reviews
• Provide health education and counseling to patients
• Collaborate with primary care providers and other healthcare professionals to ensure seamless care coordination
• Document patient information and outcomes in a timely and accurate manner

• Masters degree in Nursing or related field
• Current RN and NP licensure in state of practice
• Board certification in a Matrix-approved specialty
• Minimum of 1 year experience as a Nurse Practitioner
• Strong computer skills and familiarity with electronic health records
