Role Overview

The Lead Physician will deliver direct clinical services to a designated panel of members. This role involves guiding fellow clinicians on best practices for providing value-based care. Additionally, the Lead Physician will collaborate closely with our Medical Director to offer clinical leadership to our healthcare providers, nurses, behavioral health specialists, and medical assistants.

Key Responsibilities

  • Deliver exemplary primary care to a panel of members, leading an interdisciplinary care team.
  • Mentor fellow providers in delivering outstanding primary, urgent, or community care to their members, and effectively lead their interdisciplinary teams. Support care teams during population health meetings to achieve quality and utilization objectives.
  • Serve as the clinical leader in the center alongside the Medical Director, addressing inquiries and providing clinical guidance to nurses, medical assistants, and fellow providers regarding clinical cases as they arise.
  • Be an integral part of the center and market's leadership team, accountable for steering the market towards achieving its quality, utilization, and engagement goals. Participate in leadership meetings and represent AbsoluteCare to community organizations, hospital systems, and payer partners.

Duties and Responsibilities

  • Provide primary care services to a designated panel of members.
  • Collaborate in a team environment with social work, care management, behavioral health, nursing, pharmacy, and nutrition, facilitating discussions and implementing care plans from other team members.
  • Coach and mentor physicians and advanced practice providers.
  • Treat acute exacerbations of chronic conditions on-site.
  • Accept same-day or next-day urgent appointments for patients needing intensified physician or case management services to ensure resolution of acute episodes and prevent admissions.
  • Demonstrate the ability to assess when exacerbations can be managed within AbsoluteCare versus when a higher level of care is necessary.
  • Work with the Medical Director to establish partnerships with academic programs and community organizations to enhance service delivery to members.
  • Monitor patients and their disease processes vigilantly to prevent emergency room visits, hospital admissions, and readmissions.
  • Practice evidence-based medicine in accordance with the Patient-Centered Medical Home (PCMH) model of care.
  • Track and achieve specified quality metric goals.
  • Perform additional duties as required consistent with chronic care and disease management services.
  • Implement care plans in collaboration with patients, their families, and other team members.
  • Develop robust transitional care plans and maintain oversight of paneled patients throughout the continuum of care, including hospital discharges, emergency room visits, skilled nursing facilities, and home care.
  • Utilize technology and electronic health records to enhance quality initiatives and efficiency within the practice.
  • Participate in multidisciplinary meetings and recommend treatment initiatives aimed at maintaining patient health or resolving ongoing clinical issues.
  • Provide on-call coverage for the office in rotation with other providers.
  • Develop clinical protocols and pathways to empower all clinical support staff to practice at the top of their license.
  • Dedicate approximately 70-80% of time to direct clinical care.
  • Perform other duties as assigned.

Minimum Qualifications

  • Active, unrestricted medical license in the state of employment.
  • 5+ years of clinical and administrative experience leading interdisciplinary teams.
  • Experience in clinic settings, community-based organizations, and inpatient units.
  • Experience in training and mentoring healthcare professionals.
  • Excellent interpersonal communication skills.
  • Strong problem-solving, project management, and organizational skills.
  • Independent self-starter and critical thinker with an interest in population health and healthcare utilization management.

Working Conditions

This position operates in a professional office environment, utilizing general office equipment. The role may require wearing specific uniforms, such as lab coats or scrubs, and using protective attire, including gloves and masks. Exposure to patient diseases and infections is a daily occurrence. The position may involve travel to various centers or corporate offices as needed.

Physical Requirements

  • Ability to communicate clearly and exchange accurate information consistently.
  • Ability to remain stationary for extended periods.
  • Constantly operates computers, keyboards, copy and fax machines, phones, and other general office equipment.
