About the Role
We're seeking experienced Nurse Practitioners to join our HouseCalls team. As a HouseCalls Nurse Practitioner, you'll conduct in-home health assessments on enrolled Medicare Advantage members. Your expertise will help identify and address health risks, ensuring our patients receive the best possible care.
Key Responsibilities
- Conduct in-home assessments, averaging 45-60 minutes in length, to identify health risks and develop personalized care plans.
- Review patients' medical history, medication lists, and perform evidence-based practice screenings to inform care decisions.
- Communicate findings with patients' primary care physicians to ensure seamless care coordination.
- Identify opportunities for clinical referrals and intervene in urgent and emergent situations.
- Educate patients on disease processes, medication, and compliance to promote healthy behaviors.
Requirements
- NP license and national certification (ANCC or AANP) or ability to obtain within state of assignment.
- Family, Geriatrics, Adult, Adult-Gerontology Primary or Adult-Gerontology Acute Care certification.
- Access to reliable transportation for in-home visits.
Preferred Qualifications
- 1+ years of clinical experience in highest level of education, clinical setting.
- Experience in gerontology, cardiology, internal medicine, or endocrinology.
- Home Health care or home visit experience.
