Private Duty Nurse LPN

We are seeking a skilled and compassionate Tracheostomy and Ventilator Care Nurse to provide high-quality patient care in a private duty setting. As a valued member of our team, you will have the opportunity to make a meaningful difference in the lives of our clients and their families.
Responsibilities:- Deliver and coordinate quality patient care in compliance with physician orders.
- Continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as necessary.
- Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner.
- Participate in and update the nursing care plan to ensure effective patient outcomes.
- Take appropriate nursing action based on assessment and achieve expected outcomes.
- Recognize changes in patient needs and responses requiring intervention and implement care to prevent risk or reduce risk.
- Accept responsibility for personal and professional accountability by complying with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
- Provide care utilizing infection control measures that protect both staff and patient according to OSHA standards.
- Educate patients and families regarding the disease process, self-care techniques, and prevention strategies, and in meeting the patient's nursing needs.
- Maintain knowledge of competencies related to the nursing profession by participating in educational programs, continued education units, internal learning management skills, and skill evaluations.
- Current Pennsylvania State Nursing License in good standing.
- Current BLS CPR Certification.
- Ability to pass State & Federal Background Checks/Clearances.
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
