Job Summary

We are seeking a skilled Physician to join our team as a Primary Care Physician - Home-Based Practice. As a member of our team, you will have the opportunity to provide exceptional patient care and experience the rewards of delivering healthcare in a unique and challenging environment.

Responsibilities
  • Deliver primary health care and patient care management in a residential setting
  • Provide primary care to high acuity patients in their home Monday - Friday
  • Collaborate with our interdisciplinary team to ensure seamless care coordination
Qualifications
  • Experience in Geriatrics or complex patients preferred
  • Active/unrestricted medical license and DEA
  • Board Certified or Board Eligible
  • EMR experience
