Bilingual Adult/Family Nurse Practitioner

At Public Health Management Corporation, we are committed to delivering exceptional healthcare services to our community. We are seeking a highly skilled and compassionate Bilingual Adult/Family Nurse Practitioner to join our team.

Job Summary

The Bilingual Adult/Family Nurse Practitioner will provide primary and urgent care services to adults and/or families within our Federally Qualified Health Center (FQHC) Network. This role requires a strong commitment to delivering high-quality patient care, upholding our health center's mission and quality standards, and contributing to the operational and fiscal capacity of our organization.

Key Responsibilities
  • Provide primary care services to adults and/or families
  • Work collaboratively with our Network Clinical Director and other healthcare professionals to ensure seamless care delivery
  • Manage acute and chronic health problems using evidence-based protocols
  • Provide health screenings, immunizations, and disease prevention programs
  • Advance the role of advanced practice nurses in underserved areas through advocacy and professional activities
  • Notify immediate supervisor of significant problems
  • Provide patient/family education, health counseling, and health advocacy services
  • Be willing to work occasional weekends
  • Provide age-appropriate immunizations
  • Collaborate with community health nurses on care and follow-up
  • Participate in public health management of communicable diseases in shelters and other health initiatives
  • Maintain positive relationships with funding sources and community organizations
  • Perform other related tasks and duties as directed and assigned
Requirements
  • Ability to engage resistant clients
  • Extensive knowledge of current healthcare needs of homeless and mentally ill adults
  • Ability to collect adequate client data, conduct physical exams, interpret physical findings, and manage or refer patients as conditions or protocols dictate
  • Good verbal communication and writing skills
  • Computer skills
  • Ability to communicate and work with homeless, mentally ill adults
  • Ability to prioritize and organize workday to meet changing needs of program and clients
  • Ability to work within a team framework and respond positively to team members' concerns
  • Ability to track work to completion and compile reports
  • Bilingual in Spanish and English
Education and Experience

Must be licensed as an RN and APN Nurse Practitioner. Salary is commensurate with education and experience, and an excellent benefits package is offered.
