Qualifications:
A minimum of one year of experience in public or private social services; or at least two years of case management experience, including one year of conducting assessments of individuals' functional capabilities to ascertain the necessity for institutional or community-based services; and a Bachelor's degree, supplemented by a minimum of 12 semester hours in sociology, social welfare, psychology, gerontology, or other relevant social sciences; Alternatively, a Bachelor's degree with a major in social welfare; Or any equivalent combination of experience or training, along with successful completion of at least 12 semester hours in sociology, social welfare, psychology, gerontology, or other related social sciences.
a. One year of experience in a local Area Agency on Aging (AAA) office may be considered equivalent to one year of assessment experience.Skills Required:
Exceptional organizational, written, and verbal communication abilities; capacity to engage courteously and effectively with diverse audiences, including the disability community; adeptness at managing pressure and juggling multiple tasks; ability to work autonomously; experience with data entry into automated information systems; familiarity with local healthcare networks and providers; and the ability to uphold cultural sensitivity.
Candidates must be prepared to travel within their designated county and conduct in-home assessments. Further details regarding the Enrollment Case Manager position will be provided in the attached job description.
