Position Summary:
The Treatment Coordinator plays a crucial role in managing the comprehensive care experience for a designated group of clients. Collaborating closely with the Program Director, Counselors, Recovery Support Staff, and Clinical Aides, the Treatment Coordinator engages directly with clients to formulate and assess treatment, relapse prevention, and ongoing care strategies aimed at fostering sustainable recovery.
Key Responsibilities:
1. Develop individualized treatment plans in collaboration with clients.
2. Monitor and evaluate client progress and adjust plans as necessary.
3. Facilitate communication among team members to ensure cohesive care delivery.
4. Provide support and resources to clients to enhance their recovery journey.
Qualifications:
Possesses a solid understanding of client-centered, strengths-based counseling, case management, and treatment planning in accordance with ASAM Criteria. Exceptional communication, writing, and organizational abilities are essential, along with proficiency in Microsoft Office applications.
Certification as an Addictions Counselor or a relevant professional license is required, or the candidate must be eligible to obtain such credentials within the designated timeframe. A minimum of 12 months of continuous sobriety is required for candidates in recovery from substance use disorders. Candidates must also pass DHS Schedule L background checks and meet employment eligibility criteria. Bilingual candidates are preferred.
