Job Summary

We are seeking a skilled Registered Nurse to join our IDD team as an Exempt Hourly RN. The successful candidate will provide comprehensive nursing services to individuals with disabilities, promoting person-centered outcomes and ensuring quality of life.

Responsibilities
  • Provide assessment, treatment, and care as directed by provider order and established treatment plans.
  • Act as primary RN in the coordination of nursing services for assigned caseload.
  • Ensure timely and accurate dissemination of participant health-related information to pertinent staff, programs, and families/advocates.
  • Participate as an active member of the IDT to ensure two-way communication and promote continuity of nursing services.
  • Provide emergency/acute evaluation and intervention as necessary.
  • Advocate for all needed treatment to ensure quality of life and ensure appropriate follow-through/follow-up by associated parties.
  • Ensure surveillance of health status and appropriateness of nursing services provided to assigned caseload.
  • Complete and maintain annual and semi-annual nursing assessments, health specific protocols, plans of nursing service, and other required medical documentation.
  • Provide instruction and in-service training regarding participant health issues.
  • Promote universal precautions and model appropriate infection control practices.
  • Serve as department liaison for related workgroups/committees.
Requirements
  • Bachelor's Degree in Nursing or related field with New York State licensure to practice as a Registered Nurse.
  • OR Associates Degree in Nursing with New York State licensure to practice as a Registered Nurse.
  • Professional experience in skilled nursing facility strongly preferred.
  • Valid driver's license preferred, acceptable driving record as established by the agency's insurance carrier, use of a personal vehicle, and willingness to travel required.
