Physician Job Description

At Theoria Medical, we're seeking a skilled Physician to join our team as a Medical Director. As a key member of our post-acute care team, you'll be responsible for providing high-quality patient care and leading our medical initiatives.

Key Responsibilities:
  • Lead medical directorship functions, including quality assurance and process improvement meetings
  • Collaborate with Nurse Practitioners and Physician Assistants to manage resident care
  • Provide comprehensive primary care services, including diagnosing and treating acute and chronic illnesses
  • Document patient care in Theoria's proprietary EHR system, ChartEasyTM
  • Participate in Theoria's Chronic Care Management, Transitional Care, RPM, and other medical quality initiatives
Requirements:
  • Must be able to supervise collaborating nurse practitioners and physicians assistants
  • Must be able to utilize electronic/online medical tools, including EHR systems and state/county medical systems
