Clinical Pharmacist

Kettering Health is seeking a highly skilled Clinical Pharmacist to join our team in the management of patients receiving care at our infusion centers. As a key member of our healthcare team, the successful candidate will collaborate with nurses, physicians, and other clinicians to ensure safe, effective, and efficient delivery of quality care.
Key Responsibilities- Monitor patient treatment medications daily for appropriateness based on current clinical guidelines.
- Assess all medication orders to ensure they meet established standards and practice guidelines of Kettering Health Network and Infusion Centers.
- Report order discrepancies to appropriate staff promptly.
- Utilize computerized systems to process medication orders, document interventions, and safety reports to maintain and support safe and consistent medication administration.
- Correlate patient laboratory results to prescribed therapy, assessing need for dosing changes based on pharmacodynamic or pharmacokinetic alterations in drug disposition.
- Report concerns and suggestions to clinical staff in a timely and effective manner to ensure patient safety and positive clinical outcomes.
- Evaluate and reconcile all current patient medications, including over-the-counter and herbal medications, for drug interactions.
- Educate patients and their families to ensure their understanding of the goal, administration schedule, and potential adverse reactions to treatment.
- Rigorously maintain current patient information files and records in the pharmacy department to maintain and support safe and consistent drug administration.
- Complete required training and competencies as defined by leadership.
- Advise clinicians on drug selection based on current clinical guidelines.
- Participate in nursing education activities.
- Prepare documents for Pharmacy and Therapeutics Committee review as needed.
- Participate in performance improvement activities to evaluate the safety, effectiveness, and outcome of services delivered.
- Order and stock medication and supplies following 340B protocols.
- Supervise the preparation and distribution of medication.
- Supervise the activities of the pharmacy technician.
- Identify, recommend treatment of, and report adverse drug reactions.
- Identify and report therapy errors.
- Train other pharmacists, technicians, and students.
- Assist in the development of policy and procedures.
- Seek out and apply new pharmaceutical concepts and information as available.
- Disseminate current information to support other professional staff.
- Compound sterile products according to current guidelines (ASHP, USP, NIOSH).
- Attend any infusion-related reactions and assist with managing any acute symptoms.
- Adhere to safety requirements in the handling and preparation of chemotherapy and other hazardous agents.
- Current pharmacy licensure by the state of Ohio.
- Maintenance of continuing education as required by law.
- Graduation from an accredited college of pharmacy (BS Pharmacy or Doctor of Pharmacy).
- Preferred completion of PGY1 residency or 3 years of hospital pharmacy experience.
