Job Description

We are seeking a skilled Triage RN to join our team at North Country HealthCare. The successful candidate will have a strong nursing background and excellent communication skills, with a focus on providing clear, safe, and effective telephone triage using evidence-based processes and tools.

The Triage RN will be responsible for screening and triaging patient/staff phone calls, collecting and analyzing information, and creating a well-documented plan of care for each patient. This role requires excellent communication and organizational skills, as well as the ability to work independently and as part of a team.

The ideal candidate will have a minimum of 3+ years of experience in med/surg, palliative, hospice, and/or home care, and a current nursing license. Additionally, they must have access to an operable, insured motor vehicle and a valid driver's license.

Salary and Benefits

The salary range for this position is $62,000 - $72,000 per year, depending on experience. In addition to a comprehensive benefits package, which includes medical, dental, and vision insurance, paid time off, and retirement savings plan, you will also receive a generous sign-on bonus and relocation assistance.
