Job Overview

We are seeking a skilled Home Health Licensed Practical Nurse to provide direct patient care in the comfort of their own homes.

As a Home Health LPN, you will be responsible for delivering high-quality nursing care to patients in need of intermittent skilled nursing services.

Our goal is to help patients achieve a faster recovery, regain independence, and prevent rehospitalizations.

This is a full-time position based out of our Columbia Home Health office, covering Maury and surrounding county areas.

Key Responsibilities:

  • Provide direct patient care in the home setting
  • Develop and implement individualized care plans
  • Collaborate with healthcare teams to ensure seamless care
  • Monitor patient progress and adjust care plans as needed

Requirements:

  • Current, valid, and unencumbered Tennessee LPN license or Compact license
  • Current and valid driver license, transportation, and auto insurance
  • One year clinical experience preferred
  • Self-motivated and customer service oriented
  • Must be able to pass background and drug screening
