UPMC Chartwell is seeking a skilled and dedicated Infusion Nurse to join their Ambulatory Infusion Team. As a key member of the team, you will provide high-quality nursing care to patients in the comfort of their own homes.
Responsibilities- Coordinate and perform skilled nursing care for patients in the home setting
- Participate in teaching patients and significant others home medical management
- Provide leadership by working cooperatively with staff to establish priorities and goals of patient care
- Participate in Chartwell Pennsylvania's Performance Improvement Program
- Represent self in a professional and ethical manner at all times
- Attend scheduled patient care conferences, in-services, departmental meetings, etc.
- Take on call responsibility as assigned to provide 24 hour availability
- Support the marketing efforts of the center by assisting the sales team and providing educational programs to referral sources, as requested
- Set high goals or standards of performance for self
- Display a neat, clean, professional appearance at all times
- Participate in the activities of the Performance Improvement Program
- Submit all required patient care documentation and reports in a timely manner
- Take professional responsibility for self-protection during required work in potentially hazardous situations
- Maintain courteous demeanor at all times
- Promote Chartwell culture by providing quality, comprehensive services to all customers through a team approach
- Demonstrate ability to communicate effectively and express ideas clearly
- Make sure that work meets quality standards
- Provide infusion and other skilled nursing care to patients
- Show pride in work
- Maintain confidentiality of patient information
- Communicate with others in a warm and helpful manner while simultaneously building credibility and rapport
- Complete work on schedule
- Comply with all applicable company policies and procedures
- Complete all required patient care documentation and reports in a timely manner
- Maintain professional licensure, certification and health requirements as well as demonstrating clinical competence of orientation programs, etc.
- Take initiative to present ideas/suggestions to management
- Provide pre-discharge patient assessments when appropriate
- Participate in the orientation of other nurses or center personnel, as requested
- Demonstrate reliability and follow-through on all assigned tasks
- Communicate to supervisor, physicians and other health care providers, as appropriate, any changes in the patient's condition
- Utilize appropriate communication lines in relaying problems, concerns, questions, and ideas
- Demonstrate ability to work effectively and maintain expected productivity
- Diploma from an accredited school of nursing required
- B.S.N. preferred
- 1 year previous specialized home health experience preferred
- 1 year clinical experience involving infusion therapy, including patient assessment skills, and venous access skills
- CPR certification required
- PICC training preferred
- Excellent interpersonal, communication and organizational skills required
- Ability to assume a flexible work schedule
- CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
- Automotive Insurance
- Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
- Driver's License
- Registered Nurse (RN)
