POSITION SUMMARY

This part-time role is essential in delivering direct support to clients enrolled in our Outpatient Treatment (OT) and Intensive Outpatient Programs (IOP). The counselor will assist clients in cultivating and sustaining healthy lifestyles and effective recovery management skills.

The position necessitates a comprehensive understanding of the clinic's treatment philosophy, admission processes, and eligibility criteria. A foundational knowledge of substance use disorders and co-occurring conditions is critical. The counselor will ensure that both clinical and administrative services are effectively provided to patients and their families.

Key Responsibilities:

  1. Manage a designated caseload in compliance with regulatory standards.
  2. Ensure that all incoming clients receive a complete psychosocial history and a thorough assessment within five days of admission.
  3. Collaborate with the team to develop a Preliminary Treatment Plan within three days of admission.
  4. Facilitate the creation of a Comprehensive Treatment Plan for each patient within 30 days of admission, with active participation from the team and the client.
  5. Conduct Treatment Plan Updates every 30 days following the initial Comprehensive Treatment Plan.
  6. Prepare aftercare plans for patients completing the program or being discharged for other reasons.
  7. Provide follow-up care and maintain contact with all discharged patients.
  8. Deliver individual counseling sessions focused on addiction recovery as outlined in the Comprehensive Treatment Plan.
  9. Coordinate Health Home services in alignment with the Treatment Plan.

Qualifications:

Candidates must possess trainee status from the Board of Professional Counselors and Therapists or hold certification as a licensed addiction counselor. Licensure as an LGPC, LCPC, or CAC or higher is required.
