Job Opportunity: Home Health RN

We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Health RN. As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to deliver comprehensive care.

Responsibilities:
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and education.
  • Conduct thorough assessments of patients' physical and emotional needs, developing personalized care plans to meet their unique requirements.
  • Collaborate with our interdisciplinary team, including therapists, social workers, and other healthcare professionals, to ensure seamless care coordination.
  • Communicate effectively with patients, families, and caregivers to ensure understanding and compliance with care plans.
  • Document patient progress and outcomes accurately and thoroughly, maintaining up-to-date records and reports.
Requirements:
  • 1 year of experience in skilled home health care.
  • Current BLS certification.
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
  • Strong organizational and time management skills, with ability to prioritize tasks and manage multiple responsibilities.
  • Valid driver's license and reliable transportation.
