We are seeking a skilled Physical Therapist to join our Outpatient STARS team on a per diem basis. As a Physical Therapist, you will play a vital role in evaluating and treating patients with disabilities, injuries, and/or diseases using physical, chemical, and mechanical means.
Key Responsibilities- Evaluate patients to determine the extent of their disabilities and develop treatment plans to achieve established goals.
- Administer direct patient care in accordance with written prescriptions and/or referrals from licensed physicians or authorized practitioners.
- Maintain accurate and complete medical records for each patient.
- Respond to emergency procedures and participate in quality improvement chart reviews and activities.
- Collaborate with interdisciplinary staff to ensure optimal patient care and management.
- Master's Degree in Physical Therapy or equivalent combination of education and experience.
- Current licensure to practice as a Physical Therapist in New York State.
- Specialized certifications as needed.
- 1-3 years of relevant experience.
