Advocate Aurora Health is seeking a skilled Physical Therapist to join our team in a float position. As a float Physical Therapist, you will have the opportunity to work in various settings and provide high-quality patient care.
Job Summary:The Physical Therapist - Float Position is responsible for providing physical therapy services to patients in a variety of settings, including inpatient and outpatient facilities. The ideal candidate will have excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
Key Responsibilities:- Interpret physician referrals and complete evaluations utilizing appropriate methods.
- Establish a plan of care and implement individual treatment based on evaluation results and functional goals.
- Monitor and evaluate outcomes and adjust the individualized treatment plan as needed.
- Identify factors affecting patient comfort and attend to these needs.
- Plan for and complete patient discharge, including providing written home programs and referrals for community services.
- Complete all required documentation, including patient progress notes and discharge summaries.
- Basic Life Support (BLS) certification issued by the American Heart Association (AHA) within 6 months.
- Physical Therapist license issued by the state in which the team member practices (Wisconsin or Illinois).
- Bachelor's Degree in Physical Therapy.
- Intermediate computer skills.
- Excellent communication and interpersonal skills.
- Demonstrated organization, decision-making, and problem-solving skills.
The Physical Therapist - Float Position requires the ability to sit, stand, walk, lift, and squat throughout the workday. The position also requires the ability to push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present.
Exposed to electrical and chemical hazards as well as blood and body fluids; therefore, must have ability to wear protective clothing as needed.
Position may require travel; therefore, may be exposed to road and weather hazards.
Operates all equipment necessary to perform the job.
