**Job Description**
We are seeking a skilled Clinical Care Partner to join our interdisciplinary team at Grand Lake Health System. As a valued member of our clinical staff, you will have the opportunity to make a positive impact on the lives of our patients and their families. Your expertise and compassion will be instrumental in providing high-quality patient care and contributing to a supportive and collaborative work environment.

**Key Responsibilities:**
  • Assist with daily patient care activities, including vital sign measurements, personal hygiene, and mobility assistance.
  • Collaborate with healthcare professionals to develop and implement individualized care plans.
  • Provide emotional support and comfort to patients and their families.
  • Document accurate and timely information in patient records.
  • Participate in training and professional development opportunities to enhance skills and knowledge.


**Requirements:**
  • High school diploma or equivalent required.
  • Certified Nursing Assistant (CNA) certification or completion of an approved nursing assistant program preferred.
  • One quarter or semester of clinical experience in a healthcare setting.
