We are seeking a skilled LPN Annual Wellness Nurse to join our team at WellSpan Health. As a key member of our ambulatory care team, you will play a vital role in delivering high-quality patient care and promoting preventive health services.
Key Responsibilities- Identify and engage Medicare patients in need of Annual Wellness Visits (AWVs) and facilitate the completion of these visits in a timely and efficient manner.
- Collaborate with primary care providers and other healthcare team members to ensure seamless patient care and coordination.
- Assess patient needs and develop plans to address any gaps in care, working closely with patients to schedule follow-up appointments as needed.
- Utilize critical thinking skills and expertise in the nursing process to provide high-quality patient care and services.
- Participate in quality improvement initiatives and contribute to the development of best practices in preventive care.
- Document all patient interactions and care provided in the electronic medical record.
- Certificate in LPN program completion.
- 1 year of relevant experience in a healthcare setting.
- Current licensure as a Licensed Practical Nurse (LPN) in the state of Pennsylvania or ability to obtain licensure upon hire.
- Basic Life Support (BLS) certification.
- Excellent interpersonal and communication skills.
- Ability to work independently and as part of a team.
- Self-motivated and dependable individual with a strong work ethic.
