Independent Optometrist

Job Summary

As an Independent Optometrist leasing space inside our Walmart stores, you will take a leading role in patient care with respect to eye and vision care as well as general health and well-being. This is a unique opportunity to establish your own practice while leveraging the resources and support of a leading retail company.

Key Responsibilities

• Provide high-quality eye care services to patients in a retail setting
• Establish your own schedule and fee structure
• Have access to equipment and instrumentation for a fair market value
• Option to opt-in for administrative and/or Opt-Tech support services (if state allows)

Requirements

• Doctor of Optometry (OD) from an accredited optometry school
• Valid state optometry license
• Ophthalmologist (MD or DO) from an accredited school
• Valid state ophthalmology license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9
