Community Case Manager

As a Community Case Manager, you will play a vital role in providing case management services to assigned clients, including reviewing and determining client needs, providing case management, counseling services, and performing various professional and technical duties in support of assigned areas.
Key Responsibilities:- Conduct initial interviews with clients to gather information about their needs and circumstances.
- Assess client needs and develop individualized plans to address their challenges.
- Collaborate with community agencies and resources to provide clients with access to necessary services and support.
- Provide case management services, including counseling, advocacy, and referrals to community resources.
- Document client progress and maintain accurate records of case management activities.
- Bachelor's degree in a related field, such as social work, psychology, or sociology.
- One year of experience in case management, social services, or a related field.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Knowledge of community resources and services.
The Community Case Manager will work in a fast-paced environment, interacting with clients, community agencies, and other stakeholders. The position requires strong organizational and time management skills, as well as the ability to work effectively in a team environment.
