Job Summary

HCA Florida Kendall Hospital is seeking a skilled RN Case Manager to join our Rehabilitation team. As a key member of our care coordination team, you will play a vital role in ensuring our patients receive high-quality, patient-centered care.

Responsibilities

  • Provide program orientation to patients and families, including the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
  • Act as the coordinator of patient and family education, promoting participation in team discussions related to plans, goals, and status.
  • Ensure the implementation of the patient's treatment plan, supporting their strengths, abilities, needs, and preferences.
  • Document the findings of the Discharge Planning Evaluation and psychosocial assessments, communicating the social, financial, and discharge needs and preferences of the patient and family.

Requirements

  • RN with current state licensure; BSN preferred.
  • Three years' clinical, hospital nursing experience required, with a preference given to those with 2 years of case management experience or 2 years of critical care experience.
  • Certification in case management or utilization review preferred.
  • InterQual experience preferred.

HCA Florida Kendall Hospital is a 417-bed acute care facility located in west Miami-Dade County. Our services include trauma, burn, surgical, women's and children's services, behavioral health, and diagnostic services. We have two full-service free-standing ER facilities and are a Level I Adult and Pediatric Trauma Center certified by the Florida Department of Health.
