Job Summary


The Support Coordinator/Service Coordinator/Case Manager provides vital case management activities for clients assigned to the consolidated care team. Key responsibilities include developing individualized care management plans, brokering and coordinating services, and monitoring clinical outcomes. This role assists in locating, coordinating, and monitoring needed services and supports through the development and implementation of the Individual Support Plan (ISP).

Essential Duties and Responsibilities
- Complete assessments to identify participant needs and desires for service delivery.
- Develop Person-Centered Service Plans for each participant and modify at least annually or when there is a significant medical or social change.
- Ensure participant choice of providers by providing information for the participant to make a fully informed decision.
- Report all incidents of participant abuse, neglect, or exploitation immediately to supervisor/director.
