We are seeking an experienced and compassionate Registered Nurse (RN) to join our team as a Home Health Nurse Specialist. In this role, you will provide skilled nursing care to patients in their homes, working collaboratively with other healthcare professionals to achieve optimal patient outcomes.

Responsibilities:

  • Conduct comprehensive assessments of patients' physical, emotional, and social needs
  • Develop and implement individualized care plans that address patients' unique needs and goals
  • Provide direct patient care, including administering medications, wound care, and IV therapy
  • Collaborate with physicians, therapists, and other healthcare professionals to ensure seamless care coordination

Requirements:
