About the Role

Mercy Home Health is a leading provider of home health care services, committed to delivering exceptional patient outcomes and experiences. As a Registered Nurse - Home Health Specialist, you will play a vital role in our mission to bring compassionate care to patients in their own homes.

Key Responsibilities
  • Accept and manage referrals from acute care settings, physician clinics, and other venues to provide home health care services.
  • Maintain a strong presence in these settings, delivering excellent customer service to discharge planners, medical social workers, physicians, and patients.
  • Facilitate timely discharges and support the system initiative for continuity of care within the Mercy Health System.
Requirements

To be successful in this role, you will need:

  • A current RN license in Missouri.
  • Minimum three years of nursing experience, with a preference for experience in hospice and home health.
  • Excellent observation, nursing assessment, and communication skills.
  • Ability to provide written documentation in a timely manner.
  • Self-directed with the ability to work with little supervision.
