About the Role

We are seeking a skilled Registered Nurse to join our Home Care and Hospice team at Duke Health. As a nurse in this role, you will provide high-quality patient care in the comfort of patients' homes or in our inpatient hospice facility.

Key Responsibilities
  • Provide Comprehensive Patient Care

Assess patients' needs, develop and implement individualized care plans, and provide ongoing care and support to patients and their families.

Collaborate with Healthcare Team

Work closely with physicians, social workers, and other healthcare professionals to ensure comprehensive care and services are provided to patients.

Document Patient Care

Accurately and thoroughly document patient care, including assessments, interventions, and outcomes, in accordance with Duke Health policies and regulatory standards.

Communicate Effectively

Communicate clearly and effectively with patients, families, and healthcare team members to ensure seamless care coordination and patient satisfaction.

Participate in Quality Improvement

Contribute to quality improvement initiatives and participate in ongoing education and training to enhance clinical skills and knowledge.

Requirements
  • Education and Experience

Associate Degree or Diploma in Nursing required; Bachelor's Degree in Nursing preferred. Two years of med-surgical, critical care, or home health/hospice/infusion experience required.

Licensure and Certification

Current North Carolina RN licensure or compact RN licensure in participating state required. Current State of North Carolina Cardio-Pulmonary Resuscitation Certificate (CPR) (American Heart Association only) required.

Skills and Abilities

Working knowledge of Medicare and Medicaid regulations, including JCAHO and DFS standards, ICD-9 Coding. Ability to assess nursing needs of acute and chronically ill patients and their families. Ability to communicate clearly and effectively with patients, families, and healthcare team members.
