Transformative Opportunity:

Alumus is seeking a dedicated Nurse Practitioner to redefine the future of home-based primary care in Portland, Oregon. As a key member of our team, you will have the time needed to provide thorough, personalized care to patients in their homes.

Key Responsibilities:
  • Perform comprehensive patient assessments and collaborate with facility staff on preventive services and early changes in condition recognition.
  • Manage acute and chronic conditions, including medication adjustments.
  • Order and interpret diagnostic tests.
  • Develop, implement, and modify treatment plans as needed.
  • Participate in quality assurance and improvement activities.
  • Provide clinical education and training for patients, families, and facility staff.

Requirements:
  • Masters degree in Nursing or RN license.
  • Advanced Practice Nursing license.
  • NP Board Certification from recognized certifying body.
  • Credentialed with insurance companies preferred.
  • Fingerprint clearance.
  • Unrestricted prescribing and dispensing privileges with DEA licensure required.
  • Driving is a requirement for this position.
  • Current state driver's license and proof of insurance is required.
