Job Title: Travel Licensed Practical Nurse

We are seeking a skilled and compassionate Travel Licensed Practical Nurse to join our team at a skilled nursing and rehabilitation unit in a long-term care facility. As a Travel LPN, you will provide high-quality patient care and work collaboratively with our interdisciplinary team to ensure the best possible outcomes for our residents.

Job Summary:

This is a 13-week travel assignment with a 36-hour workweek, including every other weekend and holiday rotation. You will be working on a skilled nursing and rehabilitation unit, providing primary care and total patient care to up to 20 residents. The ideal candidate will have experience in long-term care, skilled nursing, and rehabilitation, with a strong background in med/surg and acute care.

Responsibilities:
  • Provide direct patient care to up to 20 residents, including IV medications and wound care
  • Work collaboratively with the interdisciplinary team to develop and implement individualized care plans
  • Conduct thorough assessments and evaluations to identify resident needs and develop effective care plans
  • Administer medications and treatments as ordered by the physician
  • Document resident progress and care provided in the medical record
  • Participate in quality improvement initiatives and contribute to a positive and supportive work environment
Requirements:
  • Current MO license
  • 1 year minimum experience in long-term care, skilled nursing, or rehabilitation
  • Experience with IV medications and wound care
  • BLS certification
  • IV certificate
  • Strong med/surg and acute care experience preferred
  • Dementia training and experience highly recommended
