Job Summary

At Lorain County Health & Dentistry, we are seeking a highly skilled and compassionate Nurse Practitioner to join our team. As a key member of our healthcare team, you will provide high-quality, patient-centered care to our diverse patient population.

Responsibilities

• Conduct thorough patient assessments, including reviewing medical history, eliciting relevant information, and examining patients using medical equipment and techniques.
• Develop and implement individualized treatment plans, including ordering and performing tests, diagnostic images, and other inquisitive diagnostics as needed.
• Diagnose and define treatment plans for all issues or potential issues, referring patients to specialists as needed.
• Prescribe medications and perform necessary office procedures as defined and permitted in scope of practice.
• Develop positive, professional, and long-term relationships with patients, educating them on recommended techniques to improve and maintain good health.
• Promote good health throughout the facility in conversation and behavior, referring patients to specialty care if needed for complex medical or psychological conditions.
• Collaborate with the provider team, participate in Peer Review, and maintain accurate and up-to-date patient records.

Requirements

• Master's of Science in Nursing with Advanced Practice Certification required.
• Board certification in area of practice required.
• BLS certification required.
• Licensure in the State of Ohio and meeting all requirements for credentialing and privileging required.
• Continuing education requirements as established by the State Board of Medical Examiners must be maintained.

Preferred Qualifications

• A minimum of three years of experience in a non-profit community healthcare environment preferred.
• Interest or previous experience in providing MAT/MOUD services preferred.
• Proficient computer skills and ability to become proficient in software programs specific to Lorain County Health & Dentistry, such as NextGen, EPM, EHR, etc.
• Ability to serve as a patient advocate and be a role model for change.
• Excellent communication skills with the ability to master a therapeutic communication approach.
• Ability to independently coordinate multiple tasks and work cooperatively with others.
• Bilingual Spanish speaking a plus.
• Valid driver's license and satisfactory driving record required.
