Job Title: Aftercare Case Manager

Job Summary:

The Aftercare Case Manager is responsible for providing comprehensive case management services to individuals in the TRAILS program and Sullivan House facility. This includes assessment, service delivery, and ongoing support to ensure successful reintegration into society.

Key Responsibilities:
  • Develop and maintain professional relationships with community treatment providers and employers.
  • Facilitate education sessions, classes, and groups to promote job readiness and life skills.
  • Maintain tracking data in Excel spreadsheets and work with protected information, including HIPAA and 42 CFR protected data.
  • Develop, monitor, and maintain individual service plans for aftercare participants and Sullivan House individuals.
  • Act as a liaison to community members and organizations, representing the facility in a professional manner.
  • Collaborate with Clinical and Sullivan House teams to ensure inmate goals are being met.
  • Maintain records and notes according to program procedures and facility policies.
Requirements:
  • Bachelor's Degree in education, rehabilitation counseling, or human services from an accredited college or university.
  • Thorough knowledge of correctional institution operations, knowledge of criminal codes, and facility rules and regulations.
  • Ability to use computers, manage difficult people under stress, and communicate effectively.
  • Current New Hampshire driver's license and pass a physical examination and drug screen.
