Key Responsibilities- Conduct comprehensive admission and discharge risk assessments to ensure quality, financial, and regulatory compliance.
- Negotiate and procure services and resources needed by patients, utilizing a multidisciplinary team approach.
- Monitor patient care and utilization of resources throughout the hospital stay, ensuring appropriate levels of care and cost outcomes.
- Perform transition care planning activities to secure post-hospital care arrangements and monitor patient progress.
- Participate in performance improvement initiatives and contribute to the development of quality and cost outcomes.
- 3+ years of current Case Management experience.
- Ability to perform comprehensive admission and discharge risk assessments.
- Knowledge of CMS, appeal rights, and DPH regulations.
- Experience with utilization management, discharge planning, and third-party insurance review.
- Covid vaccination strongly encouraged.
