We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to provide high-quality home healthcare services to pediatric and adult patients in their own homes. As an LPN Homecare Specialist, you will work with patients who require skilled nursing care and services, including those with complex medical conditions and disabilities.
Responsibilities:
- Utilize the nursing process to assess, plan, implement, and evaluate patient care.
- Provide skilled nursing care and services in accordance with prescribed orders.
- Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition.
- Collect, analyze, and interpret data and information from healthcare members and document actual and/or potential nursing diagnoses.
Requirements:
- Currently licensed as an LPN/LVN in the state in which the LPN will practice.
- Current TB or Chest X-Ray.
- Current BLS card.
- One year prior professional nursing experience preferred.
