About the Role

We're seeking a dedicated and compassionate Nurse Practitioner or Physician Assistant to join our outpatient practice in Puyallup, Washington. As a key member of our care team, you will play a critical role in delivering high-quality patient care and contributing to the growth and success of our organization.

Key Responsibilities

Provide comprehensive primary care to our geriatric patient population, leveraging our coaching and integrated behavioral health models to engage patients in care and drive behavior change.
Partner with the care team to develop and implement personalized care plans, utilizing our proprietary Electronic Health Record to maximize impact on the population.
Participate in weekly team-building sessions and shared discussions of challenging cases, fostering a collaborative and supportive environment.
Collaborate with the medical director to drive practice innovation and improve patient outcomes.

Requirements

At least 2 years of primary care experience required.
Currently licensed or ability to obtain licensure in the state of Washington.
Excellent communication and interpersonal skills, with the ability to build successful relationships with team members and patients.
Board Certification in Internal or Family Medicine.
