We are seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Case Manager. As a key member of our healthcare team, you will play a vital role in ensuring the highest quality patient care and outcomes.

Key Responsibilities
  • Coordinate patient care plans to promote optimal health outcomes and efficient resource utilization.
  • Collaborate with multidisciplinary teams to develop and implement effective discharge plans.
  • Identify patients at risk for adverse outcomes and develop strategies to mitigate these risks.
  • Conduct comprehensive assessments of patients' medical, psychosocial, and discharge needs.
  • Partner with Social Services to ensure post-acute medical needs are met.
Requirements
  • Associate Degree in Nursing or Nursing Diploma required.
  • Bachelor's Degree in Nursing preferred.
  • 2+ years of experience in case management or 3+ years of experience in clinical nursing required.
