Job Summary

The Licensed Practical Nurse (LPN) is a vital member of our healthcare team, providing high-quality patient care in a dynamic and supportive environment. Key Responsibilities
  • Provide direct patient care, including administering medications, performing treatments, and documenting patient information.
  • Collaborate with the healthcare team to develop and implement individualized care plans.
  • Conduct thorough assessments of patients, identifying their physical and emotional needs.
  • Develop and implement strategies to promote patient safety and prevent complications.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements
  • Graduate of an accredited LPN program.
  • Current licensure as an LPN in the state of practice.
  • Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with patients, families, and healthcare team members.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
  • Basic Life Support (BLS) certification.
Preferred Qualifications
  • Experience working in a healthcare setting, preferably in a community living center or similar environment.
  • Knowledge of electronic medical records (EMRs) and other healthcare technology.
  • Ability to work flexible hours, including evenings, weekends, and holidays.
